Can chiropractic help my TMJ? 

TMJ

The temporomandibular joint (TMJ) is the connection point between the mandible (lower jaw) and the temporal bone of the skull.This joint contains a small, but IMPORTANT, articular disc that allows for smooth opening and closing of the jaw and slight rotational movements. Ligaments and muscles help stabilize the joint and prevent excessive movement. Essential muscle groups that help in closing, opening, and side to side movements include the medial and lateral pterygoid muscles, temporalis, and masseter muscles.

TemporoMandibular Disorders

When people say they have TMJ what they really mean is they have temporomandibular joint disorder or (TMD). An estimated 10 million Americans suffer from TMJ disorders (TMD) and it affects women more than men (USDHHS). According to the National Institute of Dental and Craniofacial Research the following symptoms may signal TMD:  

  1. pain in the jaw or chewing muscles, 

  2. pain that spreads to the face or neck

  3. jaw stiffness

  4. limited movement or locking of the jaw

  5. painful clicking

  6. Popping 

  7. Grating

  8. ringing in the ears

  9. Dizziness

  10. Malocclusion - a change in the way the upper and lower teeth fit together. 

TMD can usually be traced back to muscular imbalances, acute trauma, and/or arthritis (degenerative or inflammatory). Muscular imbalances may be hard to detect without a proper evaluation and can be a secondary issue caused by misalignment of the TMJ. Acute trauma can include impact to the jaw or traumatic dental/orthodontic work. Degenerative arthritis may develop due to old trauma or be caused by wear and tear over a period of time. Subluxation or misalignment of the TMJ places excessive pressure on one disc/joint compared to the other. Subluxations of the upper cervical spine can also contribute to misalignment of the TMJ. 

Treatment

We recommend exhausting all conservative treatments before considering high cost, high risk treatments like surgery. The US Dept. of Human and Health Services suggests “it is important to avoid, when possible, permanent changes in your bite or jaw.” Meaning, surgery or implants should be considered as a last resort only! They also recommend “Look for a healthcare provider who understands musculoskeletal disorders…” making Chiropractic and physical therapy top choices for TMD. 

Gonstead trained chiropractors not only have extensive training on determination of TMD, but also on gentle, corrective adjustments when necessary. The analysis is used to determine the root cause of the TMD, whether it is in the jaw or from the upper cervical spine. The adjustment is to target the subluxation or misalignment in a safe, specific, and effective manner.

Home Treatment 

There are several self-care practices that may help in easing symptoms related to TMD. First, avoid all excessive jaw movements that exacerbate your TMD symptoms. This may include yawning, eating tough foods/taking large bites of food, or singing. Instead, try eating soft foods in smaller bites and limit the motion related to yawning, talking, or singing. Second, Stretching or release of the lateral pterygoid muscles may also provide temporary relief. Third, reduce inflammation with a cool back and ease tightness with a warm compress. Please note that tightness often indicates weakness and heat may not be indicated. The jaw and trigeminal nerve is very sensitive to temperature. On January 3rd, 2017 my wife, Rachel, and I were in a serious car crash. You can imagine my horror when I was informed the impact fractured her neck and required a surgical fusion to avoid complete paralysis.
